That sounds like a rather odd set of issues...

That is the "save to file" option? Is this repeatable and if so what browser are you using on what platform?
Which browser and version of surgeweb is this running? I did make some tweaks very recently to make the attachment downloads work more reliably in mobile browsers in particular, but these to work fine in desktop browsers too...
No particular specific suggestion as a browser level feature like this can't have anything to do with surgeweb.
As this seems like an odd set of issues I would start by clearing the browser cache, and restarting the browser or even restarting the computer and making sure you are not out of diskspace.
